BOOKINGS

All bookings are subject to the following terms and conditions. Appointments are subject to availability and a £10.00 non-refundable booking fee, which is then deducted from the final price. 

 

The remainder will be payable at the time of the appointment. Bookings are provisional until the booking fee is received. If you are unable or unwilling to make a deposit to secure the booking then with regret we cannot book the appointment for you. 

There is a minimum appointment spend of £25.00 for all bookings within a 3 mile radius from SM4 and £35.00 for outside the 3 mile radius. Depending on your location a mileage fee may apply.

Certain treatments require a patch test at least 48 hours before an appointment, these include: brow & lash tints, eyelash lift & brow lamination.

A charge of £5.00 for a patch test kit is payable. This will be posted to you as soon as your booking has been confirmed.

PARKING

All pricing excludes the cost of parking. Parking must be provided outside the main entrance to the property: any associated parking and congestion charges will be added to the cost of the treatments. You must notify us of any special parking restrictions in operation.

CLIENT CONSULTATION

Your consultation form is a confidential document and its contents will not be disclosed to third parties unless otherwise stated or in the event of fraud, illegal behaviour, etc. Your consultation form may be used for marketing purposes if you opt in to receiving information from us. If your circumstances change (including name, address, contact details, medical conditions, etc) you must inform us. We take no responsibility for any errors or problems arising from a failure to advise us of a change.

MEDICAL CONDITIONS

There are some medical conditions that are contra-indicated for some treatments. This means that you may need to obtain permission from your GP, consultant or other medical practitioner before any treatment can be carried out, or we may avoid certain areas of the body during treatment. Conditions that may be contraindicated are listed below, but this is not a comprehensive list and you should enquire about specific conditions prior to treatment:

 

High blood pressure; Contagious or infectious skin diseases; Areas of unexplained lumps; bumps or severe pain; Bone breaks and fractures; Cancer Disorders of the circulatory and lymphatic system; Skin problems such as rashes, bruises, sores, lesions, swelling, burns, boils and blisters; Heart defects and conditions including angina; Varicose Veins and Ulcers; Phlebitis; Thrombosis; Diabetes; Recent operations and / or scar tissue; Areas of inflammation and / or bruising; Slow release injections / patches; Pregnancy (especially first trimester or where there are complications); Current cold / fever Infectious illness or disease; Hernia Osteoporosis; Chronic / severe asthma; Allergies; Use of certain drugs; Recent skin treatments. 

YOUR APPOINTMENT

Please allow at least an extra 30mins for your booking, to complete your consultation form if you are a first time client and to set up and clear away afterwards.

 

We will not carry out a treatment without a completed consultation form.

 

We reserve the right to alter, refuse or terminate a treatment, without refund, in the event of: a client declaring a previously undisclosed medical condition that contraindicates their treatment (please see below for more information about medical conditions) a client being under the influence of alcohol, drugs, etc a client engaging in inappropriate behaviour.

 

Certain treatments are contraindicated by some medical conditions. In some circumstance a client may waive their rights by signing a disclaimer stating that they are happy to continue with treatment despite being advised that there is a potential contraindication. It is a client’s responsibility to raise medical conditions with us at the time of booking so that we can confirm the requested treatment is not contraindicated.  We are  under no obligation to carry out a treatment that requires a patch test if one has not been carried out with us. In some circumstance a client may waive their rights by signing a disclaimer stating that they are happy to continue with treatment without a patch test.  
 

CANCELLATIONS AND REARRANGEMENTS

We operate a 24 hour cancellation/rearrangement policy. If you cancel or ask to reschedule your appointment more than 24 hours before your appointment time, you will not forfeit your deposit. Instead it will be held as a prepayment credit to use for your next appointment. 

 

If you cancel or ask to reschedule your appointment less than 24 hours before your appointment, a cancellation fee of 50% of treatment cost is payable before being able to book in for another treatment. 

 

Appointments needs to be rescheduled and booked within 14 days from the original appointment date. If the appointment is not rescheduled and booked within 14 days, it will be treated as a cancellation and a new booking fee is required.

Please note that these terms and conditions apply for appointments made for yourself or on behalf of a third party. If making a booking for a third party it is your responsibility to ensure that person can attend the appointment and is aware of the cancellation policy.

 

If you are running late, please contact us as soon as possible.  

LOYALTY CARD

Adiva Beauty believe in thanking our customers for their loyalty.

After you have spent £30 or more on a Beauty Treatment, you will receive a loyalty card which will be stamped. 

At each subsequent visit where you spend £30 or more on a treatments, you will receive a further stamp on your card.

Once you have collected your fifth stamp on your card and it's full it can then be redeemed for 50% off 1 treatment.

 

Terms & Conditions
The Loyalty Card is not transferable
The Loyalty Card remains the property of Adiva Beauty which reserves the right at any time without notice to:

1) Terminate the scheme, 

2) Decline to issue Loyalty Cards

3) On reasonable grounds withdraw or cancel the Loyalty Cards and the stamps collected, or amend or alter the terms of the scheme.

 

Reasonable grounds include but are not limited to any abuse or attempted abuse of the scheme, use of the Loyalty Card in breach of these terms and conditions or any reasonable suspicion of dishonesty on the part of a Loyalty Card holder.
 

To collect Loyalty stamps, your Loyalty Card must be presented at the time of purchase.
You must declare at the time of booking that you are redeeming your Loyalty Card with us
Your loyalty card cannot be used in conjunction with gift voucher payment or any offers and deals.
Your loyalty card if redeemed for a treatment, does not qualify as a treatment on your new loyalty card.
We cannot reimburse the stamps on your loyalty card if it is lost, stolen or damaged
Adiva Beauty does not accept liability for lost, stolen or damaged cards
 

The Loyalty Card has no monetary value and is therefore non-refundable and non-exchangeable
